The Ontario Health Insurance Plan (OHIP) currently covers one cycle of in-vitro fertilisation (IVF) per patient. We can share more information on fertility treatment options and OHIP coverage during a consultation.
For fertility care services covered under OHIP, please fax a referral letter from your primary care physician to our office at: (905) 668-5561.
For fertility care services not covered by OHIP, you may self-refer by filling out the form below.
